Exit Interview Form

This two page exit interview form is to be used by any supervisors, managers or human resource departments when conducting an exit interview. This purpose of this form is to conduct a final interview with an employee prior to their employment being terminated either voluntarily or involuntarily. The information captured in this form can be used to determine the reason for leaving, how improvements can be made in that particular position, and whether the employee is eligible for rehire.
